Why do game makers make games and game systems that only work in certain regions?

Like for the xbox 360 they have Japanese and US version (and others, I’m sure), but the Japanese games won’t work with the US xbox 360 and the US games won’t work with the Japanese xbox 360. Why did they do this?
Do Japanese video game makers still make games with legitimate nudity in them?

I know that in America, games with the “AO” rating, which entail full nudity and related sexual content, have been banned for a few years now and therefore aren’t made anymore. However, what about the video games that are Japan-exclusive? Have they followed the rule as Americans have, or are they still making games that would be considered “AO”?
